| At the
Community Health Network our goals go beyond just providing you with health care. To
understand San Franciscans needs better, we make a special effort to create a bond with
you and your community.
Part of this bond is providing education to our patients, helping everyone to understand that good health begins in childhood and is achievable regardless of your financial or ethnic background. We stress the importance of preventive care, and encourage families to promote the importance of good health within the home. We also encourage our patients to be active, knowledgeable and to become partners with their providers in managing their personal healthcare. In 1997, the Community Health Network embarked on an organizational restructuring plan to meet the challenges of a rapidly evolving health care environment. Established as the division of the Department of Public Health that encompasses all personal health care services, the CHN has the unique role of addressing the broad health needs of all San Franciscans, with a special emphasis and commitment to serving the Citys most vulnerable, diverse populations. The CHN encompasses a wide array of services across a continuum of care. Major service components include primary care (provided at 18 sites throughout the City), specialty care, acute care home care, long term care, and emergency care. Perhaps the best way to look at the Community Health Network is this: Only a complete network of city health care providers and resources such as the Community Health Network, can truly care for a city with the diverse needs of San Francisco. Thats why we feel were a better choice to help you than any other healthcare provider.
